The Connecticut Charter Oak Health Plan provides affordable group health insurance rates to uninsured individuals. Depending on the enrollees choice of 3 participating insurers, the maximum monthly premium range is $251 to $259. Further, those enrolees with modest incomes may pay as little as $75.00 per month with an annual deductible of $150.00.
Some examples of benefits include–Primary care office visits with $25.00 copay; Specialist office visits with $35.00 co pay; Ambulance 100% covered in emergencies; Prescription medication with three tiered co-pay as low as $10.00 with a $7500.00 annual benefit. Maternity pre and post care 100% covered; Inpatient hospital with 90% covered after deductible met; Outpatient surgical 80% covered after deductible met; Emergency room visit $100.00 copay(waived if emergency) Lifetime maximum benefit is $1 million; Annual maximum benefit is $100,000.00
Income guidelines with Charter Oak premiums and annual deductibles are available at http://www.charteroakhealthplan.com/.( For example the monthly premium for a household of 1 whose annual income is over $31,200.00 would be $259.00. & the monthly premium for a household of 1 whose annual income is under $15, 600.00 would be $75.00)
Simply fill out the Charter Oak Quick Start Application. You may download it from www.charteroakhealthplan.com or call 1-877-77-CTOAK. Mail the application to the post office box on the application. You will be notified by mail of the premium and deductible cost estimate, along with an enrollment form to join one of the 3 Charter Oak insurers. Insurance coverage will begin on the 1st of the month going forward, depending on the timeframe of application filing, processing, and enrollment selection by the applicant.
Customer service and support is available at 1-877-77-CTOAK Monday through Thursday 9am to 8 pm; Friday 9am to 6 pm; and Saturday 10 am to 2 pm.

Attention Stafford Residents
Beginning on or about Monday June 30 traffic will shift along New City Road approaching the New City Road Bridge at Virginia Lane in both directions. The lane shift will facilitate the final removal and replacement of the bridge over the New City Brook. The road will remain open during construction with temporary closures of up to ten minutes. Alternating one way traffic with stop signs and flaggers will be in use daily. Motorists are asked to please use caution, observe the 5 mph posted speed limit or seek an alternate route for the duration of the summer months.

The Stafford High School Running Track is temporarily closed due to resurficing.
The track will be closed starting today June 5, 2008 and will remain closed until approximatly June 20, 2008.

PRESS RELEASE
June is Dog License Month
All dogs age six months or older must be licensed during the month of June in accordance
with state law. The cost is $8.00 for spayed or neutered dogs and $19.00 for male or female dogs. A current rabies certificate is required.
Look for the salmon postcard license renewal form which will be mailed on June 1 to the owners of all dogs registered in Stafford during the past year. If you did not receive one, or if you have a new dog, you still must comply with the license regulations. You may purchase your dog license in person at the Town Clerks’ office during regular office hours or by mail.
Please direct mail registrations to Town Clerk, PO Box 11, Stafford Springs CT 06076.
Include the following information: name, address and telephone number of the owner; name of dog, breed, age, color and sex of the dog, the required rabies and spay/neuter documentation if applicable, a self addressed stamped envelope and required fee.
Mail registrants must be postmarked by June 30th to avoid penalty fees.
State law requires penalty charge of $1.00 per month to be added when licenses are not renewed by June 30, 2008.
